Support Coordination

What is Support Coordination?

There are three levels of support coordination that can be included in your plan:

1. Support connection – We can support you to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals.

2. Support coordination – coordination of supports: We can support with which it will assist you to build the skills you need to understand, implement and use your plan. Our support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in the community.

3. Specialist support coordination – This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensure consistent delivery of service. Depending on your individual goals, plan objectives and aspirations we can assist you to achieve the objectives in your plan.

How can we help you with your Support Coordination?

We will support you to understand and implement the funded support in your plan and link you to community, mainstream and other government services. We will focus on supporting you to build skills and direct your life as well as connect you to other providers.

We will assist you to negotiate with providers about what they will offer you and how much it will cost out of your plan. We will ensure service agreements and service bookings are completed. We will help build your ability to exercise choice and control, to coordinate supports and access your local community.

We can also assist you in planning ahead to prepare for your plan review.

We will assist you to 'optimise' your plan ensuring that you are getting the most out of your funded support.

How can we help you with your Support Coordination?

10 Minute Call


A 10  minute phone consultation aimed at understanding the service needs.

Formal Meeting


A formal meeting is booked to understand the participants needs and plan and service requirement and sign Service Agreement.



Plan options are then discussed and liaison with other service providers, NDIA and community organisations


It’s support to help you understand and get the most out of your plan Depending on your individual goals and plan you may receive funding for a support coordinator. Support begins once a plan has been approved, and from there you’ll work together to unpack your plan, decide how to spend your funds to get the most value, and choose the providers that best meet your needs.

To get the help of a support coordinator, ask for it to be included in your plan at your planning meeting. But before that, it’s worth taking the time to note the reasons WHY you need the extra support, so you are well prepared.

Participants may want to explore the Home and Living Options with registered SIL properties

We can facilitate Therapist like OT, Physio, Speech for your functional Assessment or ongoing Capacity Building Requirement

4 Community Participation

Community Participation

These supports enable a participant to engage in community, social or recreational activities.

5 Support Coordination

Support Coordination

Assists you in navigating the NDIS, by connecting and coordinating mainstream, informal and community funded supports.

6 transport assistance

Transport Assistance

Assistance when travelling around your community, you can pay for a support worker to drive you to your location or accompany you in your travel around the community

7 Advocacy


We can assist you with your NDIS Application, plan review, reports or budget concerns.

8 Plan Management

Plan Management

gives you freedom from account paperwork and access your budget with the login portal
